CATARACT SURGERY

WHAT IS CATARACT SURGERY?

Cataract surgery is the lens replacement surgery required when a cataract occurs in the eye. This is when the natural lens has lost its clarity and has become cloudy. The cataract causes blurred vision, loss of contrast and can make driving difficult. Cataracts can occur at any age but are most common in people over 60 years old, due to the natural ageing process of the eye. Cataracts can also be genetic, or caused by other factors such as diabetes or previous eye trauma.

If left untreated cataracts normally become larger, obstructing vision more over time, and eventually lead to blindness.

What is corneal transplant surgery?

Corneal transplantation refers to replacement of all or part of the cornea; the specific transplant type is dependent on the underlying problem.

There are many different reasons why the cornea may be damaged and lose its clarity and transparency leading to loss of vision. These can include infective causes such as the herpes simplex virus (cold sore virus) or contact lens related infection, traumatic injuries, inherited diseases such as Fuchs’ dystrophy or lattice dystrophy, or even previous cataract or glaucoma surgery.

The optical function of the cornea may also be affected by conditions that lead to a change in it’s shape including keratoconus or pellucid marginal degeneration, or due to prior infection or injury.

Often, instead of transplanting the entire cornea, only the diseased portion is transplanted. In keratoconus, this means leaving the endothelial cell layer undisturbed and only transplanting the anterior (front) part of the cornea (Deep anterior lamellar keratoplasty or DALK).

Treatment types are varied including penetrating keratoplasty, lamellar surgery or selective endothelial keratoplasty; your surgeon will recommend the best treatment option for you. What is the Tear Duct?

Did you know that your tear duct plays an important role not only in making you cry but also in draining those tears away? A tiny passageway called the nasolacrimal duct sits below your eyes, allowing the tears to flow from your eyes to your nose. However, when this duct is narrowed or blocked, your eyes can get excessively watery, leading to tears dripping down your cheeks. Sometimes, you might also notice extra mucus or crust around your eyes, indicating a problem with the tear duct. In severe cases, a blocked tear duct can even lead to an infection in the area between your eye and nose, so keep an eye out (pun intended) for any unusual symptoms!

Is all tearing and watering of the eyes due to a blocked tear duct?

While a blocked tear duct can certainly cause tearing, there are other potential causes of tearing including:

  • Dry eyes
  • Allergies
  • Infections
  • Corneal diseases
  • Eye surgery
  • Eyelid malpositions (e.g. ectropion or entropion)
  • Lashes touching the surface of the eye (trichiasis)
  • Eyelid lesions
  • Other causes

Common nonsurgical dry eye disease treatments include:

  • artificial tears
  • prescription eye drops that help your eyes produce more tears
  • warm compresses or other ointments or drops to help treat underlying conditions that may cause dry eyes

Sometimes these treatments aren’t enough. If your eyes still feel dry and uncomfortable, your doctor may recommend one of the following dry eye surgeries.

Which is better—an endoscopic DCR or an external DCR?

When it comes to surgical treatment for tear duct issues, the endoscopic DCR and external DCR are two options to consider. While most doctors prefer the endoscopic DCR due to its less invasive approach, faster recovery time, and ability to perform during active infections, the external DCR is still a viable option for patients who are very sick, have unique anatomy, or take blood thinners. Plus, the scar from an external DCR generally fades over time and is barely visible, particularly in older patients. Overall, both procedures have their advantages and should be discussed with your doctor to determine which is the best fit for your individual needs.

WHAT IS RECOVERY AFTER TEAR DUCT SURGERY?

Endoscopic DCR offers a quick and hassle-free recovery with minimal bruising and swelling. The patients may have a bit of blood-tinged drainage from the nose for up to 48 hours, but there is typically no pain. Most patients can even walk on the same day of surgery and do light activities such as using their PC and watching TV. Just remember to avoid heavy lifting and bending for the first two weeks. As part of the recovery process, you will need to use antibiotic drops for the eye and a steroid nasal spray (Flonase) for the first two weeks. Also, nasal saline sprays will need to be used several times a day to help maintain clean nasal passageways, which can speed up the healing process and improve breathing. The stent will be removed after 2-3 months, and the tearing should improve within just one week.
